I have worked with people who have mental health, learning disabilities, brain acquired injuries, dementia, parkinson, physical disabilities, substance misuse and the frail and elderly with numerous ailments. I have over 8 years experience assessing individuals in the community who have care and support needs. I will complete assessment and devise a care and support plan. I'm also experienced with completing safeguarding concerns and enquires. I have experience in ordering aids and adaptions and reviewing equipment. I have worked collaboratively with social workers, physiotherapist and occupational therapist for the same desired outcomes. I understand the process of completing referrals for additional support and am well converse with the Care Act 2014. I embrace the Equalities Act and also promote diversity. I am passionate about providing care and support for individuals that need it. I feel that I have great job satisfaction when I have the opportunity to make a difference in someone’s life. I love meeting new people, eating out, using technology and travelling to new places. I love to watch black & white screen movies and True Movies.
