 LS11 0AS 282872 I have been working within the Hospital and Care sector for the past many months and have gained experience within a variety of directorates. Over the past year, I have been working with Agencies who specially allocate me to work at various Surgical, Respiratory, Mental ( General Medicine) Wards and Care Homes, several of the patients have presented with a mental health diagnosis which has impacted on their ability to make sense of the area they are in and as a result of this they have displayed challenging behaviour that has been both verbal and physical. Most of the patients who have demonstrated this behaviour have been deemed not to have capacity and have either been under a DOLs or on occasions they have needed to be sectioned under section 2 of the mental health act. I have also supported individuals who have had a diagnosis of Bi-Polar Disorder, Autism, Epilepsy and also Schizophrenia and I have recognized the need to have boundaries in place which need to be reinforced on a regular basis and all staff have needed to be consistent in what they are saying in order to prevent any form of manipulation, It is also vital that we remain professional with our service users in mental health and maintain professional boundaries resisting the possible need to self-disclose personal information. We are there as a support in a professional capacity not as a friend. I have worked with Doctors, Physiotherapy, Nurses among others as I have gained vast knowledge about delivering good quality care for patient. I understand that patients have the right to exercise their choice and live in an environment free from abuse, most importantly be involved in any safeguarding decisions made furthermore, all care must be person centered as this uphold dignity, kindness, compassion, respect and human right. It is important that we recognize that each patient will have varying needs and that any care that is planned is both holistic and person centered as just because there may be 3 individuals who have the same diagnosis that does not mean that they share the same experience. Within the planning of care we should be able to look at situations such as the patient who wanted a cigarette and wherever possible include them in the formulating of the plan setting firm boundaries. I have a very calm manner and this has proven to be an asset when working with individuals with challenging behaviour as I have found that this changes the atmosphere and I utilize distraction in order to take their mind away from whatever has caused the escalation in behaviour. I have excellent communication skill both verbally and in regards to documentation and utilize online documentation. I am aware that if things are not documented then they did not happen and if a patient is in a heightened state they may forget what they have said or done so accurate records are vital. I work well as a team member but I also have the initiative to undertake aspects of the role on my own however I would never work outside of my remit. I have experience of working in partnership with service users and other organisations to ensure a holistic approach in identifying and meeting the needs of the service users. I ensure that their views and wishes are part of all decision making process therefore working in an anti-oppressive way.   I am able to utilise the resource in the community, through sign posting service users to relevant agencies and providing service users with information thereby empowering them and enabling self-determination. I have excellent skills in partnership building, acknowledging some of the complex needs service user might have. I value the input all the organisations involved with service users to ensure the holistic assessment and meeting the needs of the service users.  In doing so am aware of confidentiality boundaries and I ensure that I share information appropriately in accordance with the Data protection Act (1998). Due to varying nature of individual needs, I have experience of working in a person centred way taking into account race, culture, age , gender, ethnicity, religious view and ensuring that service user’s needs are meet, thereby acknowledging diversity which is reflected in the community. I am able to communicate effectively using different communication skills that are relevant to the service user’s needs. I am able to facilitate and understand the importance of using language that is jargon free to enable service users to understand the communications. Adapting different methods of communication to meet the needs of the service users has been instrumental towards building trust, honest and good relationships with service users. I have experience of managing my own work load, and ensuring accurate record keeping using different data bases, in line with the organisation’s policies and procedures.  I have experience of using different IT data bases. I have a good working knowledge of Microsoft word. I am able to learn new data bases.  I am able to work with no or little supervision, however I am aware of my limitation and I am able to seek guidance and support.
