For example, there have been many instances where I have had to assess client needs independently and put in place care plans at the point of new referrals without my team being present. However, I often work collaboratively with my peers to ensure the best possible outcomes for client care. One incident that stands out was working as part of a large multi-disciplinary team around a family with multiple problems; everyone in my team contributing to the development of a support plan that utilized the strengths we all have to ensure we provided the best possible outcomes. I also have experience with Physiotherapy and I have worked with patients who required physiotherapy in the past, aiding them in their movement to prevent stiffness. I'm also happy to help out within the household whether that be cooking, cleaning, ironing etc.
